Case scenario.......

Hi, I am M Sharath chand of 3 BDS  student.This is an online elog book to discuss our patients health data after taking her consent.This also reflects my patient centered online learning portfolio.

CASE SHEET:a 18 year old female student residing in narketpally with 3 episodes of seizure from yesterday night

Chief complaints: patient was asymptomatic until yesterday night she had a first episode of seizure gained consciousness after 10 min and again after 4 hrs she had a  second episode of seziure and 3rd episode of seziure at 7 hrsand gained consciousness after 10 min





HISTORY OF PRESENT ILLNESSES 
Chest pain, fever, frontal headache non radiating vomiting and having nausea in the morning 







ASSOCIATED DISEASES: nil





PAST HISTORY: patient having fever about 20 days ago and had a episode of seziure about 1 year back and stopped medication on their own after one month






PERSONAL HISTORY:nill

This also reflects my patient centered online learning portfolio. CASE SHEET: A 32 year old male came with chief complaint of abdominal pain ,fever since 7 days and burning micturition since 3 days. CHIEF COMPLAINT: abdominal pain since 7 days  fever since 7 days  burning micturition since 3 days. HISTORY OF PRESENT ILLNESS: Patient is asymptomatic 7days ago. since 1 week he is suffering from abdominal pain. the pain is pricking type of pain, continuous, aggravated on inspiration. The pain is in left iliac and right iliac region.  fever since 7 days, on and off, high grade , intermittent, associated with chills. CASESHEET :     62 year old patient with                            severe diabetic foot infection  CHEIFCOMPLAINTS  :   left foot infection with draining fluids  since 1 month 3days HISTORY OF PRESENT ILLNESS: patient was having  type 2 diabetes since 11years.
